Montpelier Exempted Village School has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign various required forms for your child/children. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your child/children, saving you time.

 

Once you have registered and entered all required information, you will only be required to review your information once per school year to verify information is current and sign the forms.

 

We are requiring that ALL parents of students use FinalForms. Please register at – https://montpelier-oh.finalforms.com/ -- now and electronically complete ALL forms online! Resources are available at the school for those with limited or no internet access.

Parents of students K-2 must complete the parent forms, but K-2 students are not required to complete the student forms.

Open the Parent Playbook for information to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”.
When setting up your child/student, please be sure to use their school email address or leave blank.  Personal email addresses may not work properly with FinalForms.
